• DSPS Disability Verification Form - is the form we will send to your medical professional. If it is more convenient for you to take this form, you can submit the completed form with the application packet.
  • Completed and signed by an appropriate professional (this may be a physician, psychologist, psychiatrist, or audiologist depending on the disability type)
  -OR-
  • Include copies of any recent IEP, Psychological/Educational reports, Department of Rehabilitation documents, or Veteran Affairs documents that may by applicable
