DSPS Forms

The DSPS application documents are not automatically submitted. Please download the DSPS application forms, save them to your computer, complete the forms electronically or handwritten. You can email, fax, or mail the forms. Please contact the DSPS office at your campus to discuss the best method for you.

  • DSPS Disability Verification Form - is the form we will send to your medical professional. If it is more convenient for you to take this form, you can submit the completed form with the application packet.
    • Completed and signed by an appropriate professional (this may be a physician, psychologist, psychiatrist, or audiologist depending on the disability type)
    • -OR-
    • Include copies of any recent IEP, Psychological/Educational reports, Department of Rehabilitation documents, or Veteran Affairs documents that may by applicable
