Financial Aid Forms 2018-2019


Once you have completed the FAFSA application, the CR financial aid office may send you an email requesting additional documents. Please only submit documents that are requested.

Please be aware that turning in forms that are not required may result in longer processing times for your financial aid file! Not sure which forms you need to complete? Login to WebAdvisor and go to the 'My Documents' section to see if any documents are required from you at this time.

You may submit documents by mail to: Financial Aid Office, College of the Redwoods, 7351 Tompkins Hill Road, Eureka CA 95501. 

You may also submit a PDF of the document by sending an email from your student email account to: finaid@redwoods.edu.

 

Form Description/Information
Dependency Status Review     This optional form can be used to document extenuating circumstances regarding a student's dependency status.
Dependency Status Update This optional form can be used to update Dependency Status AFTER a Dependency Status override was approved for a previous year.

 

California College Promise Grant (formerly known as the Board of Governor's (BOG) Fee Waiver) Application

Use this form to apply for the BOG fee waiver for Fall 2018, Spring 2019 and/or Summer 2019 (for California residents, eligible AB 540 students and eligible AB 1899 students).  If you complete the FAFSA application, you are already applying for the BOG fee waiver with that application.  Please check your award letter on webadvisor PRIOR to completing the BOG fee waiver application to see if you have already been awarded a fee waiver. The BOG fee waiver does NOT cover course material or audit fees, books, health fees, or student activity fees.  Make sure you check your statement and pay any applicable fees when you register for classes .
California College Promise Grant Loss Appeal Use this form to appeal the loss of California College Promise Grant (formerly known as the BOG Fee Waiver) eligibility due to academic standing.
Verification Worksheets - V1B, V1C, V2, V3, V4, V5, V6 This link will bring you to a series of verification forms. Students who have been selected for the verification process will be notified about which specific forms that they need to complete (NOT all of the forms). The forms are coded by number and you will need to pay attention to which forms are required for you. Be sure you know which of these forms you need before printing!

Parent/Student Federal Tax Documentation Instructions

Tax documentation may be required for some students (and/or their parents).  This link provides instructions on how to get the required documentation.  Students will be notified if documentation is required.

High School Completion Status

This form is required after a FAFSA has been completed if there is a question regarding whether a student has met the Ability to Benefit Requirement of HS Graduation or equivalent. Students will be notified if this form is required. Be sure you need this form before printing!

Academic Program/Ed Goal Change Notification This form may be required after a FAFSA has been completed to ensure that a student has chosen an eligible academic program and educational goal before they receive financial aid. Students will be notified if this form is required. Be sure you need this form before printing!
Homeless Status Verification This form may be required after a FAFSA has been completed if a student indicated on the FAFSA that they are homeless and that is the only criterion which would make the student Independent (the student would otherwise need to provide parent information on the FAFSA). Be sure you need this form before printing!
Selective Service   This form may be required after a FAFSA has been completed if a student is male and has not registered for selective service. Students will be notified if this form is required. Be sure you need this form before printing!
Status Information Letter Request This form may be required after a FAFSA has been completed if a student is male and has not registered for selective service AND is over 25 years of age. Students will be notified if this form is required. Be sure you need this form before printing!

This form is used to make a change to an original loan request (either to reduce or increase the original amount requested). To cancel a Loan, please use the Financial Aid Cancellation Request Form, found at the bottom of this page.

Past Due
Proof that 2016 IRS
income tax return was not filed.
Independent students and parents of Dependent students who did not file 2016 IRS tax return(s) will be required to provide proof of non-filing.  There are a few ways to request this proof:
1.  Use the Get Transcript Online option at www.irs.gov
2.  Complete a 4506-T form and fax/mail it to the IRS, requesting option 7 for Verification of Nonfiling. (click here to view a sample completed 4506-T).


Note: You may need to submit Form 8822, Change of Address if you are submitting a 4506-T form and address lines 3 & 4 are different.
   
SAP Appeal

This link will bring you to a video tutorial that must be successfully completed before accessing the SAP Appeal form. First time users will need to register for an account to view the video. Be sure you need this form before printing. Students will be notified when this form is required!
This form may be required if a student is not meeting the Satisfactory Academic Progress (SAP) requirements (due to an excessive number of cumulative units attempted) and their financial aid has been suspended.  SAP status is re-evaluated at the end of each semester, when grades are available.

Note:  Summer 2019 SAP Appeals will be accepted from May 31, 2019 through August 9, 2019

Unusual Pell/Enrollment History Appeal This form may be required if a student has demonstrated an unusual enrollment activity over the last three academic years (multiple terms of receiving financial aid and not completing enrollment periods at multiple institutions). Be sure you need this form before printing!
Special Conditions This optional form can be used to request a recalculation of your eligibility for financial aid based on changes in your financial circumstances.  The FAFSA application and all other required FA paperwork must be completed and received by CR before this form is submitted to CR.
   
   
Student Asset Information This form may be required if student asset income information is being requested. Students will be notified when this form is required. Be sure you need this form before printing!
Parent Asset Information This form may be required if parent asset income information is being requested. Students will be notified when this form is required. Be sure you need this form before printing!
Bachelor's Degree Confirmation This form may be required if there is a need to clarify whether or not the student has earned a bachelor's degree. Students will be notified when this form is required. Be sure you need this form before printing!
Release of Information   This optional form can be used to designate the Financial Aid Office to release student financial aid information to someone other than the student. Please note that while this form allows for the release of information, it does not require us to provide information to others.
Financial Aid Cancellation Request

This optional form can be used to request a cancellation of all or a portion of a student's financial aid award(s).  Please note that there is another form listed above that is specific to loans. To reduce/cancel/increase an existing loan request, please use the Direct Loan Change Request Form.
