Verification Worksheets 2018-2019
Students who have been selected for the verification process will be notified by email about which specific forms that they need to complete (NOT all of the forms). The forms are coded by letter and number and you will need to pay attention to which forms are required for you. Use the links below to access a PDF of the form. HOW DO I VERIFY MY INCOME INFORMATION ?
In addition to completing the verification worksheet form(s) below, students and parents are asked to provide additional documentation.
If the student/parent filed a 2016 Federal Tax Return, there are two ways to document the income information:
One way to verify tax information is by using the FAFSA IRS DRT (IRS Data Retrieval Tool) on the web at www.fafsa.gov! Log into your FAFSA, select Make FAFSA Corrections, select Financial Information, select View Option to Link to the IRS.
Another option is to submit a Tax Return Transcript. To obtain a 2016 IRS Tax Return Transcript, go to www.IRS.gov and click on the "Get Transcript of Your Tax Records" link, click "Get Transcript Online" and follow the prompts to "Create an Account." Once logged in, the system will want to know why you are requesting a transcript. Select "Higher Education/Student Aid," then select "2016." (PLEASE NOTE: You must use the exact information you used when filing your 2016 IRS income tax return. For example, if you enter "St" in the Street Address, but you used "Street" on your tax return, it could prevent the IRS from finding your record). Another option for requesting a Tax Return Transcript is to call the IRS at 1-800-908-9946.
PLEASE BE AWARE THAT WE CANNOT ACCEPT YOUR TAX RETURNS (i.e. IRS 1040, 1040A, 1040EZ) AS DOCUMENTATION. THE INFORMATION MUST COME FROM THE IRS, EITHER BY DIRECT TRANSFER TO THE FAFSA OR BY A TAX RETURN TRANSCRIPT!
If the student/parent did not file a 2016 Federal Tax Return, AND is not required to file, IRS W-2 forms are required if the student/parent received W-2(s) from an employer. Proof of non-filing is also required in some cases - Independent Students, as well as parents of Dependent Students will be required to provide proof of non-filing if they did not file a tax return.
WHAT HAPPENS AFTER I SUBMIT ALL OF MY REQUIRED PAPERWORK?
Financial Aid Staff will review your verification paperwork and may need to make corrections to your FAFSA application to ensure that it is accurate and complete.
